What Is HomeFirst?
Yooralla's HomeFirst program can provide you with a range of different supports depending on what you need. Yooralla's HomeFirst also offers you the opportunity to co-manage your HomeFirst funding as a flexible package arrangement and have the opportunity to be more creative and effective with your funding.
For this to be successful, Yooralla's HomeFirst Coordinator works in partnership with you to create a flexible package within Department of Human Service (DHS) funding guidelines. To ensure you have a say in what happens, your HomeFirst Coordinator works with you to develop the following: an Individual Support Plan (ISP), a roster and an individual budget that represents your personal needs and goals. It is also the role of the HomeFirst Coordinator to support you and negotiate an effective package with DHS.
Community access
Is about finding new ways to use your local community services such as:
- public transport
- banks, shops and grocery stores
- recreational and social programs
- making those necessary appointments to your doctor and dentist
Home management
Is about gaining the confidence to manage your household needs such as:
- learning to shop
- planning interesting meals and cooking
- creating and sticking to a budget
- keeping your home safe and clean
- for some people, practicing the skills to eventually move into their own home
Personal care
Is about keeping yourself healthy and fit by developing a personalised program of your choice:
- recreational program such as going to the gym
- healthy eating plan, grooming and presentation
Eligibility
People aged between 6 and 64 who have either an intellectual disability or an acquired brain injury.
Referral and application
HomeFirst is accessed via the Department of Human Services Needs Register. An application form can be obtained from your regional Department of Human Services' Office or from a disability service provider. Registration on the Service Needs Register is through the Disability Intake, Access and Response Teams at regional offices. The disability intake worker should determine whether HomeFirst is appropriate.
Fees
This is dependent upon the service request.
