What is Respite Service Coordination?

The Respite Service Coordination Program has been designed to assist carers of a person with a disability to identify and access meaningful respite.

A Respite Service Coordinator meets with carers to discuss individual respite needs and assists in planning for future respite access.

The coordinator, in conjunction with the primary carer, will develop an individualised respite plan that meets the needs of the carer and also provides a positive experience for the person they are caring for.

Coordinators meet with carers intensively whilst developing the respite care plan, then remain in periodic contact to ensure the respite options are still meeting their needs. Should the respite no longer meet the carer and client's needs, the coordinator will work with them to review and redevelop the original respite plan.

Coordinators can assist with:

  • information about disability respite services
  • information about other community based or generic respite services
  • assistance and advice to community groups or services to make their respite service more accessible to a person with a disability
  • assistance with referrals to respite services
  • assistance in accessing funding for respite
  • assisting carers to complete documentation required to access appropriate respite services

Eligibility

To be eligible for the program you need to be the primary carer of a person who:

  • has a physical or intellectual disability, sensory impairment or an acquired brain injury
  • does not have current access to case management
  • resides in the Eastern Metropolitan Region
  • is aged between six and 64 years

Priority will be given to carers who are:

  • not currently accessing respite services
  • supporting a person with complex care needs
  • ageing
  • from a culturally or linguistically diverse background
  • are supporting a person who is needing to transition from existing respite services due to age, change of needs. to new services

Referral and Application

Parents and primary carers may contact the program coordinators directly. Case managers, service providers, GP’s or schools are all welcome to make referrals with the consent of the primary carer. Fees
